Recover Faster,
Stay Home
Home health care is skilled medical care delivered in the comfort of your own home. It's for patients recovering from surgery, illness, injury, or managing a chronic condition — people who need real clinical expertise, just not in a hospital or facility setting.
Our nurses, therapists, and aides come to you — bringing the same quality of care you'd receive in a clinic, in the environment where you're most comfortable and motivated to get better.
Home health is not the same as hospice. Hospice is comfort-focused care for life-limiting illness. Home health is recovery-focused care for people who are working toward greater independence and function.

How Home Health Works
From referral to first visit, we make the process as simple as possible. Most patients receive their first visit within 24–48 hours of referral.
Referral or Call
A physician orders home health, a discharge planner refers, or a family member calls us directly. We accept referrals from any source.
Same-Day Response
Our intake team contacts the patient or family the same day to confirm eligibility, verify insurance, and schedule the initial assessment visit.
Care Plan Created
A registered nurse visits the patient at home, conducts a full assessment, and works with our physician team to create an individualized care plan.
Care Begins
Skilled visits begin on a schedule tailored to the patient's needs — and we adjust the plan regularly as the patient progresses toward their goals.
Home Health is Covered
by Medicare
If your loved one qualifies, Medicare Part A covers home health care at 100% — no copays, no deductibles. Most Medicaid plans and private insurance policies cover it too. We verify your coverage before the first visit.
Covered by Medicare Part A
Skilled Nursing & Therapy
Nursing visits, physical therapy, occupational therapy, speech therapy, and medical social work — all covered at 100% with no copay when criteria are met.
Also Covered
Home Health Aide Services
When a patient is receiving skilled care, home health aide services for personal care may also be covered — bathing, grooming, and daily assistance.
We Accept
